Medical Summary DVM Intake Exam intact female Estimated age: 7 years Microchip noted on Intake? negative History : Owner surrender. Subjective: Obese intact female cat. Observed Behavior – Calm until she was shaved. She started jumping. Evidence of Cruelty seen – None Evidence of Trauma seen -None Objective P = 160 bpm BCS 9/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: NAF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intace female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Obese Plan Abdomen was shaved but the patient started to jump and medical staff could not confirm sex status. Prognosis: good SURGERY: Okay for surgery
