STRAY – STRAY WAIT, NO HOLD Reason STRAY
Intake condition EXAM REQ Intake Date 04/19/2017, From NY 11101, DueOut Date 04/22/2017,
Recheck cat with injuries to right front paw, dehydrated
S/O: QARH. Timid but allowed all handling. Eating well. Drank all water. Clean litter
EENT: Eyes clear, no ocular or nasal discharge, ears wnl
HL: Normal thoracic auscultation
ABD: Soft, non-tender
INTEG: Dirty hair coat, healing wound/scab at top of head ~1cm, healing linear wound with scab pealing off at intrascapular region, wound at digit 5 of RF paw lateral to nail with exposure of nail bed
MS: Muscle wasted, ambulatory x 4
A: Two healing wounds, most significant wound at RF paw, underweight — reportedly found in elevator shaft
P: Continue with current treatment and monitoring plan. Excellent prognosis
History – Stray cat found in an elevator shaft today.
Microchip scan negative.
Age est 2 yrs.
Observed behavior – Shy, calm, allows all handling with slow approach.
Evidence of cruelty – No.
Evidence of trauma – Yes.
O: QAR. mm=pink, tacky, CRT<2 s. HR=180, RR=20. BCS=2/9.
ORAL: Good dental condition.
EENT: Small amount of dried brown material at nares. No overt nasal discharge, sneezing, or congestion.
H/L: Lungs clear, no murmurs/arrhythmias.
ABD: Sift, non-painful. No masses/organomegaly.
UG: Male, intact. Both testicles palpable.
MS: Amb x 4, right front limb lameness. No palpable fractures. Generalized muscle wasting.
INTEG: Dirty haircoat. Decreased skin turgor. Scabbed wound on top of head. Linear dried wounds and scarring over dorsal interscapular area with scabbing peeling away.
Right fore – small wound digit 5, slight bleeding and scabbing. Sensitive on palpation.
A: Age est 2 yrs based on dental condition. Dehydration 5-7%. Old wounds over dorsum – r/o burn injury or other trauma. Wound on dorsum of head and right front paw likely from falling into elevator shaft.
P: Temporary waiver from neuter surgery due to injuries and underweight.
Buprenorphine 0.3 mg/ml 0.2 ml SQ administered. Continue BID x 3 days.
Convenia 80 mg/ml 0.25 ml SQ administered.
Soak paw in Chlorhexidene solution BID x 5 days.
100 ml LRS SQ administered. Continue q 24 hours x 4 more days.
