Medical Summary DVM Intake Exam Estimated age: 1-2 years based on overall size Microchip noted on Intake? Scanned negative History : Part of hoarding case, unsocialized. Bit a handler yesterday. Subjective: Due to DOH status, pt was transferred from feral den directly into a Freeman net. Pt was quiet, curled up and leaning away from examiner during the exam. No signs of aggression – no growling, hissing or spitting. Objective BAR-H, BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Unable to examine PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, some crusting discharge near the vulva. No current vulvar discharge. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally. Assessment: Vulvar discharge – R/O recently in heat vs. other Plan: Spay Prognosis: Good SURGERY: Okay for surgery 1088
