Subjective: QAR, hydrated
Observed Behavior – Friendly,allows exam and treatment with minimal restraint.
Evidence of Cruelty seen -no
Evidence of Trauma seen -no

R = wnl
EENT: clear OU, ears clean, no nasal or ocular discharge noted
Oral Exam: decidiuous dentition.
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: male intact.
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Seems sore/uncomfortable when handling, no obvious signs of trauma, generalized discomfort/vocalizing when cradled.
CNS: Mentation appropriate – no signs of neurologic abnormalities
Assessment: A little quiet, vague discomfort.

recheck in morning (appetite/attitude/etc)
continue to monitor at ACC
SURGERY: okay for surgery if BAR in morning.
Vet Notes: 9:06 AM
S/O: QARH. Allows all handling. Normal urine in litterbox, no stool, no interest in food
EENT: Pink mm, adult dentition erupting, eyes clear, no ocular or nasal discharge, ears wnl
HL: Normal thoracic auscultation
ABD: Soft, no masses or organomegaly, slightly tender but suspect not abdominal discomfort
INTEG: WNL, no wounds appreciated
MS: Ambulatory x 4 but walks gingerly, slightly hunched under and lame in hind end, uncomfortable on palpation, no palpable fractures
A: Hind end lameness, discomfort
P: Rec simbadol 0.35ml SQ q24 x 3 days and mirtazapine 1/4 tab PO tonight. Pelvic radiographs to be done today. Open prognosis pending diagnostics
Addendum: Sedated with telazol by LVT for radiographs — no fractures or wounds noted by under sedation. No fractures or dislocations seen on radiographs, thin area of air opacity at caudal stifles. R/O soft tissue injury
