09/06/2017 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 6.7 LBS.
Recheck: 1431 BAR, hydrated, appears to be eating/drinking well EENT: palpebral alopecia, otherwise clear au/ou, negative oral PLN: nsf ABD: benign U/G: female presumed spayed, Neuro: nsf H/L: 120 regular, no murmurs, ssp lungs: resps are clear and eupnic MSI: severe generalized alopecia/dermatitis and pruritis as noted before. A: severe dermatitis/pruritis: rule out allergic vs infect/inflamm vs other P: continue to monitor appetite/attitude, recommend full dermatology evaluation/diagnostics. Recommend starting on low dose prednisone for now. 2.5 mg PO BID x 3 days, then 2.5 mg PO SID x 3 days, then 2.5 mg PO EOD.

08/31/17 19:30 Jinx is a 1 year old cat who was surrendered by her owner due to severs skin issues. The owner stated that he got her from a friend when she was a week old and he has had her ever since. The owner stated that Jinx has skin allergies and possible psychogenic alopecia. The last time the cat has been to the vet for these issues was about 6 weeks ago. Owner stated that when around strangers she is very friendly and outgoing. She has spent time around children all ages and she is relaxed and affectionate. When playing with children and adults she is gentle. The owner states that since Jinx has had skin issues she has not been interested in playing. Jinx has not spent time around other cats or dogs so it is unknown how he will react. On the way to the ACC in the car Jinx was quiet and relaxed. Owner stated that she has also been peeing outside of the litter box on the floor since she has been suffering from the skin allergies. This has been going on for the past month and Jinx has not seen a vet for this issue. When being bathed she will struggle to get away. She isn’t bothered if she has her nails trimmed. Jinx enjoys being held and brushed. She isn’t bothered if she is put in a carrier or disturbed while she sleeps. Owner described Jinx as friendly affectionate and quiet with a medium activity level. He owner stated that his favorite thing about Jinx is how loving she is. Her favorite activity is to sit on the porch with her owner and bird watch. Jinx has been kept indoors only and eats dry food. When at home she will follow her owner around and she likes to sit on their lap. Jinx has not been interested in toys since she has had the skin allergies. Jinx was very much litter box trained but for the past month she has been peeing outside the litter box. She is used to an uncovered litter box with clumping litter. Owner stated that she does not scratch things in the home but she does have a scratching post made out of carpet.

Reaction when softly spoken to: Jinx makes eye contact when spoken to softly Reaction to cage door opening: Jinx remains relaxed in the front of her kennel and watches as the cage door opens. Reaction to touch: Jinx allows petting along her head and body, leaning in and head-butting the assessor’s hand. Begins to purr. Reaction to being picked up: Jinx remains calm and relaxed when picked up and held in the assessor’s arms. She continues to purr. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Sweet, Affectionate, People Oriented BEHAVIOR SUMMARY: Average Jinx interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. Scratching excessively Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = WNL BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Minimal dental wear and staining PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, owner reported as spayed, unable to confirm spay scar due to severe dermatitis MSI: Ambulatory x 4, skin free of parasites, no masses noted, severe generalized dermatitis with superficial wound around entire neck CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Severe dermatitis and pruritis, R/O allergies Plan: Rec convenia 0.3ml SQ.
