Jeff – A1115328 – 4 week female black/white kitten, OS:
Estimated age: 4 weeks
Microchip noted on Intake? No
History : owner surrender
Subjective: BAR, apparently healthy
Observed Behavior – timid at first but then easiliy handled
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
T = P = 120 R = eupneic BCS 5/9
EENT: Eyes clear, mild scleritis right eye and pupil slightly constricted, ears clean, no nasal discharge noted
Oral Exam: pink mm w/ CRT<2 s, deciduous teeth
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
apparently healthy (mild anisocoria- previous injury?)
SURGERY: Okay for surgery
