I am a female white and gray tabby domestic sh mix. The shelter thinks I am about 11 WEEKS old.
I came in the shelter as a STRAY on 11/28/2016 from NY 11368, owner surrender reason stated was STRAY.

12/01/2016 Exam Type RE-EXAM – Medical Rating is 3 C – MAJOR CONDITIONS , Behavior Rating is NONE, Weight 2.5 LBS.
12/01/16 Monitor diarrhea, appetite, hydration S/O: QAR. Mild dehydration. Appetite ok (ate about half dish kitten food). Diarrhea in litter EENT: Eyes clear, mild epiphora, crusted mucoid nasal discharge H/L: Increased bronchovesicular sounds, no crackles; normal heart sounds A: Diarrhea, URI, mild dehydration P: Rec doxycycline 0.3ml po q24 x 10 days, LRS 25ml sq q24 x 3 days and panacur 0.56ml po q24 x 3 days. Good prognosis 11/30/16 Diarrhea noted during enrichment. O: BARH. mm=pink, moist, CRT<2 s. Eating well. Pudding-like brown diarrhea in litterbox. ORAL: All deciduous teeth present. EENT: No oculonasal discharge. H/L: Lungs clear, no murmurs. ABD: Mild pot-bellied appearance. Abd soft, non-painful, no masses/organomegaly. MS: Amb x 4, good body condition. INTEG: Full, clean haircoat. Normal skin turgor. A: Diarrhea DDx: Parsitism, infectious disease, dietary change/intolerance. P: Parvo Test = negative. Rx Metronidazole susp. 100 mg/ml 0.25 ml PO q 24 hours x 5 days. First dose given. Monitor diarrhea, appetite, hydration. Excellent prognosis.

11/28/16 11:40 Jazz is a white and gray tabby DSH with no known medical condition. During intake Jazz was very tense, Jazz tried to flee and did not allow handling.

12/01/2016 BEHAVIOR EVALUATION – AVERAGE
Exam Type BEHAVIOR
Jazz was brought in as a stray, so we cannot speak to her behavior in her previous home. Reaction to assessor: Jazz was resting on her blanket at the back of the kennel. Reaction when softly spoken to: Jazz lip licks then turns towards the assessor and slow blinks. Reaction to cage door opening: Jazz remains motionless. Reaction to touch: Jazz seems unsure at first then slowly leans forward to sniff the assessor’s hand. She allows petting all over, leans in for rubs then slowly rises up and comes to the front. Her body relaxes when rubbed, she gains confidence and gives the assessor soft head butts. Reaction to being picked up: Jazz is calm, relaxed and has a soft body when held. Behavior Determination: Average Jazz interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.
