Jazz is at risk for medical reasons, having been diagnosed with Upper Respiratory Infection, which is contagious to other cats. Jazz also has corneal opacity affecting one eye, but this does not cause him any problems. Behaviorally, we have no concerns for Jazz, whom we believe would be suitable for most homes.

DVM Intake Exam Estimated age: 3-5 years based on dentition and secondary sex characteristics Microchip noted on Intake? Scanned neg – MC placed during intake History : Abandoned, no health hx available Subjective: Alert, soft body, allows all handling Objective BAR-H, MMs pink and moist, BCS 5/9 EENT: OD anterior synechia – iris is attached to the cornea in a roughly 4×8 mm region. Difficult to see the lens or fundus due to the synechia. Mild mucoid discharge OU, no conjunctival erythema, chemosis or other sign of active URI. Clean ears, no nasal discharge. Oral Exam: Missing several incisors; other incisors are angled to the sides, and there is moderate gingivitis in this region. There is no gingivitis and minimal tartar on the other teeth. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Testicles S/S. Large jowls. MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally No fluorescein stain uptake OD Assessment: 1. Anterior synechia OD – evidence of old trauma, no current disease 2. Dental disease – possible FORL vs. other cause of localized gingivitis Prognosis: Good Plan: 1. No treatment needed for right eye, but it should be monitored to ensure that there is no progression of signs 2. Recommend dental cleaning and daily tooth brushing SURGERY: Okay for surgery 1088

S/O -QAR -no obvious appetite -sniffling, mild nasal congestion -OU: mild to moderate blepharospasm, no significant discharge; OD-large corneal opacity, r/o scar vs other -appears eupnic A 1. URI/conjunctivitis 2. Corneal opacity P -cerenia 0.4 ml SQ SID x 2 days -nebulize SID x 4 days
Notes entered for 1088. S/O: Quiet in kennel today. Mild epiphora OU, no nasal discharge. A: URI resolving, suspect viral strain P: discontinue doxycycline and recheck to see if OK to move out tomorrow.
Hx: In isolation on treatment for URI and conjunctivitis. Pt has synechiae OS. S: Alert, comes to the front of the kennel to say hi O: BAR Crusty discharge OU Mild SND Sneezing A: URI not resolved P: Recheck in 2 days. Not necessary to add any treatments. 1088

KNOWN HISTORY:: Jazz was brought in as a stray, so we don’t have any behavioral history or tendencies in a home environment.
MEDICAL BEHAVIOR:: Subjective: Alert, soft body, allows all handling
ENRICHMENT NOTES:: Resting in litter box, soft eyes and body. Dozing, seemed a little lethargic. Lay in place when door opened, leaned forward to sniff my hand, then leaned into pets. Extended neck for throat scratches, closed eyes.
Cage Condition:: No change
Reaction to assessor:: Jazz remains neutral, lying down on his cage bedding during the approach.
Reaction when softly spoken to:: Jazz becomes alert with normal shape eyes.
Reaction to cage door opening:: Jazz remains immobile with ears erect and forward.
Reaction to touch:: Jazz flinches when he seeing the assessor’s hand, but slowly leans into the petting while remaining stiff in place.
Reaction to being picked up:: He becomes antsy then jumps back into the cage.
