Medical Summary DVM Intake Exam Estimated age: 3yrs Microchip noted on Intake? yes History : Subjective: returned for inappropriate urination Observed Behavior – docile allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 180 R = 50 BCS 5.5 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mild gingivitis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FS, grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment FS reported to have been urinating inappropriately as per prev o: r/o stress/sterile cystitis, UTI, other Plan convenina 80mg/ml: 0.45ml SQ once in case of UTI check urination, if inappropriate urination – consider trazadone 25mg PO SID if inappropriate urination continues- consider Dr. Elsey’s Cat Attract litter, feliway, Hill’s C/D stress diet, and rec regular daily schedule to decrease stress (play, brushing p, quality time), and rec have two litterboxes Prognosis: fair to good SURGERY: n/a
