Medical Summary DVM Intake Exam Estimated age: 1yr Microchip noted on Intake? no History : Surrendered due to owners being evicted Subjective: Observed Behavior – moves to back of carrier, seems disinterested in attention at first, then rubs into petting; nervous outside of carrier, but allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 170 R = 30 BCS 5/9 EENT: Eyes clear, ears are erythematous with flaky dark debris (very pruritic), no nasal discharge noted Oral Exam: Healthy adult dentition (Grade 0/4 dental dz) PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, bladder is small and soft, able to palpate several large-marble-sized lumps (suspect early to mid-pregnancy) U/G: see above MSI: Ambulatory x 4, skin free of parasites, no masses noted, dry flaky skin CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Suspect early to mid-pregnancy Suspect Ear Mites Plan: On 7 day Hold Trimmed nails Cleaned AU and applied Tresaderm – continue BID x 7 days, then recheck Prognosis: Good SURGERY: Okay for surgery. Recommend OVH asap due to possible pregnancy.
