GONE 7/3/17 JACKI was brought into the shelter in very bad shape. They are suggesting euthanasia. Jacki keeps falling over, she is very weak, dehydrated and depressed. We hope that she will steal the heart of someone before she is pts and perhaps have a chance at life.

DVM Intake Exam Estimated age: 13yrs Microchip noted on Intake? yes History : individual came in stating that he found cat then walked out Subjective: Observed Behavior – very weak, allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 180 R = 28 BCS 3/9 EENT: mature cataracts OU, ears clean, no nasal discharge noted, but dirt/debris caked onto front of face, mild hypersalivation Oral Exam: severe dental tartar no oral masses seen PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, grossly normal, no palpable MGTs MSI: when in cage was Ambulatory x 4, but very weak, was attempting to dig in litterbox almost falling over, skin free of parasites, no masses noted, unkempt furcoat. When took out for exam, lying laterally, too weak to stand ,severely dehydrated CNS: slightly depressed mentation: Assessment very weak, depressed, severely dehydrated, cataracts underlying metabolic/endocrine/neoplastic condition Plan offered food: no interest, but then wathed and p was eating on own given p still has interest in food and is very dehydrated, will place IV catheter10ml/hr, cbc/chem/t4- assess p’s status after starting fluids Prognosis: poor to grave SURGERY: Permanent waiver due to geriatric
