Itty Bitty A1115035 – 3 week old male black/white kitten – thin, conjunctivitis, needs syringe feeding:

Subjective: not eating when offered
Observed Behavior – vocal, active
Evidence of Cruelty seen – n
Evidence of Trauma seen – n
Objective
P = 120 R = wnl BCS 3/9
EENT: eyes sealed shut with discharge, corneas intact
Oral Exam: decid teeth
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
U/G: intact male
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Assessment: thin, not eating conjunctivitis
Plan: erythromycin ou bid, fluid support, nutrical
Prognosis: fair
SURGERY:
Temporary waiver due to age, illness
