Medical Summary DVM Intake Exam Estimated age: 12 weeks Microchip noted on Intake? no History : Found stray, no medical history Subjective: QAR, nervous, euhydrated, thin Observed Behavior – nervous but allowed all exam Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 3/9 EENT: Mild crusted discharge OU, ears clean, no nasal discharge noted Oral Exam: missing first deciduous incisors, no ulcers PLN: No enlargements noted H/L: NSR, NMA, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact male MSI: Ambulatory x 4, mild flea dirt, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: flea infestation, underweight for age/unthrifty, faint FIV combo positive Plan: monitor for URI and weight gain for 3 days prior to surgery, retest combo at 6 months – may be positive secondary to maternal antibody Prognosis: good SURGERY: Temporary waiver due to poor body condition
