ENRICHMENT:
Very active and playful
Cloudy eyes. Resting on bedding, soft eyes and body. Stood and came to the front when door opened. Leaned and arched into pets, ate offered treats.

History :owner surrender due to owner’s health concerns; horus came in with several different medications including Enalapril (2.5 mg PO SID), Clopidogrel (75 mg tab, 1/4 tab PO SID), Methimazole (2.5 mg PO BID) and Furosemide (10 mg PO BID)
Subjective:BAR
Observed Behavior -appears friendly and interactive; head butting and allows some handling but will hiss and lunge unpredictably
Evidence of Cruelty seen -n
Evidence of Trauma seen -n
Objective
BCS=5.5/9
EENT: OU-lens appears slightly cloudy; AU-clear; no nasal discharge
Oral Exam:limited exam; mm appear pk, sl tacky
PLN: No enlargements noted
H/L: continuous arrythmia; grade III heart murmur; intermittently increased resp rate (50 breaths/min) but difficult to determine whether it is behavioral; normal effort
ABD: Non painful, no masses palpated
U/G: neutered
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal:not performed

Heart disease
Hyperthryoid-based on methimazole rx
Prognosis:guarded to poor
Plan:
Previous vet records not available at this time but based on arrhythmia and multiple previously prescribed medications, I suspect Horus has advanced heart disease; will try to obtain previous records
Recommend CBC/chem/T4 to monitor renal values/T4 levels
Recommend cardiology consultation to provide best prognosis and treatment plan moving forward
Concern for behavior in the shelter as he is unpredictable and medicating may be difficult
**adjusting doses to SID during stay in shelter**
Clopidogrel 75 mg tab: 1/4 tab PO SID
Furosemide 20 mg tab: 1 tab PO SID (instead of 1/2 tab PO BID)
Methimazole 5 mg tab: 1 tab PO SId (instead of 1/2 tab PO BID)
Enalapril 2.5 mg tab: 1 tab PO SID (same dose)
