Medical Summary DVM Intake Exam 8/19/17 Estimated age: ~5weeks, male Microchip noted on Intake? No History : Stray Subjective: Observed Behavior – BARH. attention seeking, allowed all handling. Eating well Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = WNL BCS 3/9 EENT: Conjunctiviits OD, ruptured cornea OS, ears clean, mucoid nasal discharge noted Oral Exam: pink mm, deciduous teeth erupted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Underage, underweight, URI, conjunctivitis, ruptured cornea Plan: Continue to monitor. Rec doxycycline 0.13ml PO q24 x 10 days, erythromycin OU q12 x 10 days. Prognosis: Good Temporarily waived from surgery due to underage, URI
