Medical Summary DVM Intake Exam Estimated age: 5w, based on dentition Microchip noted on Intake? negative History : stray Subjective: alert, active, very loud Observed Behavior – eating well Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective EENT: ou crusted shut and mucopurulent d/c. Once eyes cleaned severe conjunctivitis and elevation of 3rd eyelids, scant waxy debris au Oral Exam: deciduous present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic, 180 bpm, ssp ABD: general sarcopenia U/G: mi MSI: Ambulatory x 4, no overt exoparasites, food caked to nose and stool on hind end CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment: 1) conjunctivitis – r/o viral vs bacterial 2) underweight 3) underage Plan: erythromycin BID x 10 days keep eyes clr of crusts and debris BID Prognosis: Good
