DVM Intake Exam
Estimated age: 8 weeks.
Microchip noted on Intake? No.
History : Litter of 2 kittens, noted to have eye and nasal discharge.
Subjective:
Observed Behavior – Active, alert, a little shy, allows all handling.
Evidence of Cruelty seen – No.
Evidence of Trauma seen – No.
Objective: BARH. mm=pink, moist, CRT<2 s. HR>240, RR=24. BCS=4/9.
ORAL: Deciduous teeth WNL, no oral lesions visible.
EENT: OD – mod chemosis, mucoid discharge. Mod. seromucoid nasal discharge, sneezing, congestion.
PLN: No enlargements noted.
H/L: Lungs clear, no murmurs/arrhythmias.
ABD: Non painful, no masses/organomegaly palpated.
U/G: Male, intact.
MSI: Ambulatory x 4, no lameness. Skin free of parasites, no masses noted. Full, scruffy haircoat.
CNS: Mentation appropriate – no signs of neurologic abnormalities.
A: 8 week old kitten with conjunctivitis, URI. Mildly underweight.
P: House in iso.
Rx Erythromycin oint. Apply OU BID x 10 days.
Rx Doxycycline 50 mg/ml. Give 0.25 ml PO q 24 hours x 10 days.
Excellent prognosis.
Temporary waiver from neuter surgery due to URI, underweight.
