Medical Summary DVM Intake Exam Estimated age: 5 years Microchip noted on Intake? History : owner surrender due to poss urinary obstruction; was treated at the ASPCA hospital over the weekend for urinary obstruction/FLUTD; owner feels that he is blocked again today and is unable to continue providing medical treatment for him Subjective: BAR Observed Behavior – sweet, curious; allows handling Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective P = 120 R = BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: mm pk, tacky; CRT <2 sec; mod-severe tartar, missing multiple incisors; tip fracture of upper canine with poss root exposure PLN: No enlargements noted H/L: no murmurs/arrhythmias ABD: soft abdomen U/G: bladder moderately distended and slightly firm; unable to manually express; tip of penis appears erythematous MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Feline lower urinary tract disease with obstruction Dental disease with fractured canine Plan Pursue sedation to place urinary catheter, rec’d keeping catheter in for 24-48 hours Abdominal rads to r/o urinary stones-rec’d cystotomy if stones present Buprenex BID x 5 days Onsior SID x 3 days Prazosin Rec’d prescription urinary diet indefinitely to control signs Prognosis: good; FLUTD is very common in neutered male cats and signs can normally be managed well long term with diet but prognosis depends on how he recovers from this acute episode; presense of urinary stones does not decrease long term prognosis but he will require surgery if they are present SURGERY: neutered
