Medical Summary DVM Intake Exam Estimated age: 1-4 years, difficult to accurately assess due to emaciated body condition Microchip noted on Intake? None History : unknown. Empt can of food in carrier. Subjective: BAR, sweet, likes rump pets, moderately prolonged skin turgor Observed Behavior – Tolerant of all medical handling Evidence of Cruelty seen – possibly neglect Evidence of Trauma seen – no Objective T = P = 160 R = eup BCS 1/9 EENT: Eyes clear, ears clean, no nasal discharge noted; bridge of nose is alepcic and hyperpigmented Oral Exam: 104 tip fx with pulp exposure, mild dental calc PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, two small fecal balls (v lns) palpable in abdomen U/G: prominant nipples, suspect IF MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt and dirty, marked generalized muscle atrophy CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NP Assessment Young adult F DSH 1. Emaciated BC r/o poor nutrition/neglect v systemic illness v other 2. Max canine fx 3. Nasal bridge alopecia and hyperpigmentation r/o chronic rubbing on cage, door, etc v other Plan -vaccinate, MC, pyrantel, -retro negative -100 ml SQ LRS tonight, repeat again tonight and then x 2 days -Vet check tomorrow to assess hydration -Montior eating very closely -CBC/Chem, TT4 if able (attempted today, unable to get as did not bleed back) -Will likely need 104 extracted due to pulp exposure Prognosis: Guarded to fair pending reason for poor body condition SURGERY: Temporary waiver due to emaciation
