Medical Summary DVM Intake Exam Estimated age: ~4-5 weeks Microchip noted on Intake? negative History : stray Subjective: BAR Observed Behavior – very sweet. Easily handleable and easy to pick up. Enjoys being held and did well for all medical handling and procedures. Purring Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = wnl R = eupneic BCS 5/9 EENT: mucoid d/c ou with no conjunctivitis, bilateral mucoid nasal d/c, clean ears Oral Exam: kitten dentition, no oral ulcers PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic but audible upper resp congestion ABD: Non painful, no masses palpated, distended U/G: FI MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Underage URI Plan: Continue to monitor while at BACC Start doxycycline 10mg/kg PO SID x10d Start erythromycin ou bid x10d Recheck day 5, 10 Prognosis: good SURGERY: temporary waiver for surgery due to underage and illness
