I came in the shelter as a STRAY on 11/28/2016 from NY 10470, owner surrender reason stated was STRAY.

Reaction to assessor: Grant looks neutral, lying down on his cage bedding when approached by the assessor. Reaction to door opening: Grant stiffens up in place and slowly retreats with ears slightly to the side. Reaction to touch: Grant accepts the stroke, but flinches during the touch then warms up and slowly head-butts the assessor’s hand while he remains wary with his surroundings. Placement determination: Experience Grant tolerates attention and petting but may be fearful or stressed in the shelter. We recommend that this cat go to a home with experienced cat parents.

12/02/16 10:02 S: eating, hisses when you reach in for him, but allows handing O: BAR H OU: mild MPOD, OD cloudy with corneal edema, OU 3rd eyelid elevant CV: NMA, S&S pulses, pink moist mm Resp, minor MPND, mild inspiratory sterdor A: URI P: cont tx move to iso px: good 12/01/16 10:47 s: seen eating canned diet on his own this morning, hissing with ears back in kennel but allows handling and exam, relaxes when pet under chin O: QAR minor skin turgor OU 3rd eyelid elevation, chemosis, OS conreal opacity, cont discharge no oral lesions noted, no resistence to mouth being opened, halitosis and staining on teeth, no hypersalivation seen no inpsiratory sterdor ausculted, no nasal discharge noted, not sneezing A: URI P: cont tx monitor appetite if eating, move to iso px: good if beging to eat, poor if contines to be anorexic 11/30/16 Did not eat this morning – when food was applied to mouth, pt licked it off, but was not interested in eating on own. Skin turgor somewhat improved from yesterday – estimated 5-8% dehydrated. Elevated third eyelids OU, diffuse corneal opacity OD. When mouth was manually opened, there was a slight click as the right maxillary and mandibular canine teeth touched. No swelling or laxity was palpated in the TMJs. Continue with current tx, monitor appetite – if appetite not improving, rec further investigation into TMJ/canine tooth alignment. Consider EHR if pt declines or does not improve. 1088 —– 11/29/16 Hx: URI, dehydration, poor appetite. Did not eat this morning. S: Sleeping. Allowed all handling but moved away from examiner. O: QAR, decreased skin turgor, estimated 8-10% dehydrated. BCS 3.5/9, MMs pale pink EENT: Elevated third eyelids OU, diffuse corneal opacity OD. Clean nose this morning. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. A: URI with conjunctivitis, dehydration, poor appetite Short-term prognosis: Fair P: Increase LRS to 60 ml tonight only. CWCT. 1088 — 11/28/16 15:46 S: came in today as as a stray O: lethargic
