N Microchip Number (If Applicable): History: ACO impound- owner died Subjective: QAR Observed Behavior – compliant for exam Evidence of Cruelty seen – N Evidence of Trauma seen – N Objective T = DNP P = WNL R = WNL BCS 3/9 EENT: Eyes clear, moderate ceruminous debris AU w/pinnal-pedal reflex, no nasal or ocular discharge noted Oral Exam: Crown fx 204, moderate dental dz (2/4 tartar) PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Suspected intact female- no visible spay scar or tattoo MSI: Underweight, ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: Externally WNL Assessment: Ear mites, underweight, dental dz; suspected intact female adult cat Prognosis: good Plan: ok for surgery and placement. Rec dental cleaning. Ivermectin 0.06ml SQ. Second dose of ivermectin in 2wks. SURGERY: Okay for surgery.

Vomiting and diarrhea reported over the last 2 days; initially had a good appetite but now reportedly inappetant S/O -BAR, friendly and interactive, very affectionate -reportedly 5-6 years old but based on dentitia and age related changes OU, suspect 10-12 years -mm lgt pk, tacky; CRT 2 sec; moderate to severe staining -no nasal discharge or sneezing -OU: open and clear, clouding of lens, iris atrophy -AU: moderate erythema, moderate to severe inflammation, yellow discharge -eupnic, no murmurs or arrhythmias noted -soft, nonpainful, doughy abdomen -severe muscle wasting A 1. v/d-r/o pancreatitis vs stress vs chronic GI disease vs other 2. Anemia 3. Geriatric, suspect underlying condition 4. Dehydration 5. Emaciated, muscle wasting 6. Otitis externa, treated for ear mites, may be residual inflammation P -move to medical for observation -100 ml SQF SID x 4 days -metronidazole 50 mg/ml: 0.3 ml PO BID x 7 days -consider IVF if dehydration persists -significance of anemia unknown at this time but due to her age and severe muscle wasting, there is concern for underlying neoplasia; rec’d further work up including CXR and abdominal ultrasound

Hx reported V/D. Transferred to medical yesterday, started on SQF SID x 4 days and flagyl. Overnight, no vomiting reported. Stools normal at noon. Continues to be very quiet but friendly. S/O -QAR, friendly and interactive, affectionate -mm lgt pk, slightly tacky; CRT 2 sec; moderate generalized pddz -no nasal discharge or sneezing -OU: open and clear, corneal opacity OU with iris atrophy -AU: moderate erythema, moderate to severe inflammation, yellow discharge -eupnic, no murmurs or arrhythmias noted -severe muscle wasting. MCS 1.5/3. BCS 3/9 A 1. v/d-r/o pancreatitis vs stress vs chronic GI disease vs other – none past 24 hours 2. Anemia 3. Geriatric, suspect underlying condition 4. Emaciated, muscle wasting 5. Mild otitis externa – appears to be improving P -continue in medical iso for monitoring. prognosis guarded to grave, as underlying condition unknown at this time. Blood sample obtained for IH CBC/Chem profile. Blood drawn from LHL medial saphenous vein with a 25g butterfly set. Cleaned ears with mild ear cleaning solution, applied tresaderm AU. Gave 0.2mL Cerenia SQ Administered 100mL SQF LRS

KNOWN HISTORY:: Grant was brought in as a stray, so we don’t have any behavioral history or tendencies in a home environment.
MEDICAL BEHAVIOR:: 1/14/18 Observed Behavior – compliant for exam
ENRICHMENT NOTES:: Vomiting noticed last two days. Crouched in main compartment, soft eyes and body, seems rundown. Moved into cubby and lay down when door opened. Leaned and arched into pets, purred. Affectionate, just not feeling well.

Reaction when softly spoken to:: Grant responds back with a meow.
Reaction to cage door opening:: Grant remains standing at the front of the cage, relaxed body posture, soliciting attention.
