KNOWN HISTORY: Gouda was brought in as a stray, so there is no information on his behavior history or tendencies in a home environment. He was very vocal during intake, but allowed all handling to be conducted. MEDICAL BEHAVIOR: Observed Behavior – very sweet. Easily handle able and easy to pick up. Enjoys being held and did well for all medical handling and procedures EVALUATION: Cage Condition: Cage is neat Reaction to assessor: Gouda looks at the assessor and is calm and relaxed. Reaction when softly spoken to: Gouda leans forward and rubs his cheeks on his blanket. Reaction to cage door opening: Gouda remains soft and relaxed. Reaction to touch: Gouda head-butts and immediately arches his back when stroked. He leans in for more rubs, begins to purr, and rolls over. Reaction to being picked up: Gouda remains soft, nuzzles against the assessor, and continues to purr when held. ACTIVITY LEVEL: Moderate VOCAL: Quiet CHARACTER TYPE: Social, Affectionate, Sweet BEHAVIOR SUMMARY: Beginner Gouda interacts with the observer, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

DVM Intake Exam Estimated age: ~8-10 weeks Microchip noted on Intake? negative History : stray Observed Behavior – very sweet. Easily handleable and easy to pick up. Enjoys being held and did well for all medical handling and procedures Evidence of Cruelty seen – no Evidence of Trauma seen – yes Subjective: QARH. Hiding in the back of the cage cowered down and dumpy. Not normal and playful. Objective P = wnl R = wnl BCS 4/9 EENT: bloody/purulent nasal d/c bilaterally, no ocular dc, mild debris in ears au Oral Exam: kitten dentition with chipped upper incisors and bruising of gums around them. Very large ~2cm lingual ulcer-very painful on oral exam and holds mouth open PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic but audible upper respiratory congestion ABD: Non painful, no masses palpated U/G: MI MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt and malodorous haircoat with thick blood covering face and nose. Diarrhea around perineum. No palpable fractures or pain on limbs, jaw, or trunk CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: URI with lingual ulcers r/o calicivirus vs other Bruising on face and upper incisors r/o high rise vs other trauma Diarrhea Plan: Start doxycycline 10mg/kg PO SID x10d Recheck daily Move to medical iso Continue simbadol 0.24mg/kg SID SQ-tech miswrite dose as BID last night but he was never given BID dosing-switched to SID Prognosis: Fair Temporary waiver for surgery due to underage and trauma
