SAFE 5/13/18 FIV POSITIVE – GEORGE was dumped at the shelter by his owner who stated his cat has not been eating, drinking or using the litterbox in 3 days. George needed to see a vet, not the shelter. Poor George is dehydrated and tested positive for FIV. He needs further medical tests to determine what is wrong. George also has a heart murmur. He is already neutered and only 5 yrs old. Give this boy a chance to feel better and have a furever home!

George has lived with two spayed female cats (2 years) and he was relaxed, gentle and affectionate around them. When George is outdoors he chases squirrels and tends to fight with other outdoor cats when they go into his territory.

Behavior Notes
George fights with cats when he is outdoors and hides when he meets new people. He allows being put into a carrier he walks right in and tends to be very quiet.George enjoys getting his coat brushed and is OK with being picked up but not for long periods of time.

George has come home with wounds about a year ago. The owners treated the scratches with neosporin and healed within a couple of days.

George hasn’t been eating or drinking in 3 days and hasn’t urinated or defecated for 3 days as well. He looks to be lethargic and loss a lot of his body weight as per owner.
For a New Family to Know
George is very friendly and affectionate. He is independent and quiet. Once comfortable around you he shares a close bond with you. He loves to go outside and sleep in the basement. He likes 9lives dry food. He uses and uncovered litter box with clay litter.
Behavior Notes:
Upon intake George seemed scared but allowed me to scan for a microchip. He walked right out of his carrier into a kennel. He has the tendency to flee if given the chance but will allow you to pick him up and place him back in his kennel.

Lying in litterbox with a low, tense body. Leans forward to sniff my hand as I reach towards him and tolerates petting along head and body while licking lips and turning head to watch my hand. Ears remain forward. Curious but unsure, needs more time to adjust. Not too interested in treats today.

George’s ears rotate and he slowly inches forward to investigate.

George remains calm.

George seems a little hesitant and inches away when approached, but he sniffs the assessor’s hand and perks up when pet. He starts to gain confidence, gives head butts and inches towards the front of the kennel with his tail lowered. He appreciates attention, but occasionally turns his head, following the assessor’s hand when pet along his back.

Abdomen (two view)- stomach appears small, intestines appears diffusely small and fluid filled, bladder moderate in size.
S/O: BARH. Timid but allowed all petting and handling, head butting at times but will run to adjacent cage with fast movements. Shows interest in food but not eating (dry, wet, baby food), offered tuna and ate well. Clean litter
EENT: Eyes clear, no ocular or nasal discharge, pink mm, moderate dental staining
HL: Grade II-III/VI heart murmur, lungs auscult clear, normal RR/RE
ABD: WNL on very brief palpation
INTEG: Full coat

Subjective: QAR. Nervous. Ate a small amount of cooked chicken (microwaved) last night. No eliminations. Food not touched this morning.

R = wnl
~5% dehydrated
CRT: <2s. Gums: pink, sl tacky
Eyes: Mild serous ocular discharge OU.
Ears: Unremarkable AU.
Nasal Cavity: No nasal discharge.
Oral Cavity: moderate dental tartar/periodontal disease with accompanying halitosis and gingivitis.
PLN: WNL
Heart: Grade II-III/VI murmur noted. Pulses adequate and synchronous.
Lungs: Eupneic, lungs clear. No crackles or wheezes bilaterally.
Abd: slightly tense, but not overtly painful. No palpable masses. No organomegaly appreciated.
U/G: Normal external genitalia. No discharge.
Musculoskeletal: Ambulatory x 4 with no appreciable lameness.
BCS = 6/9
Integument: Otherwise unremarkable haircoat.
Neuro: QAR. – no overt neurologic abnormalities
Rectal: Not performed. Externally normal.

1) Dehydration, Anorexia – r/o stress vs secondary to FIV+ vs other infectious vs inflammatory (pancreatitis, immune-mediated disease) vs primary GI disease (occult intestinal obstruction/intussusception, less likely infectious gastroenteritis vs less likely gastroenteritis (dietary indiscretion) vs IBD/lymphoma vs dietary hypersensitivity) vs systemic neoplasia
– concern for development of hepatic lipidosis given history of anorexia and lack of good appetite in hospital thus far
2) Heart Murmur- r/o underlying heart disease (HCM) vs DRVOTO
3) Dental disease- moderate
4) FIV+
5) Febrile (cyclic?)- r/o secondary to stress vs underlying condition (infectious, inflammatory, neoplastic)
Prognosis: guarded, depends upon identifying underlying condition

CTM in Medical- monitor appetite, urination, defecation, temperature
Added Mirtazapine 3.75mg PO EOD for three doses, then re-eval (5/11)
