Medical Summary DVM Intake Exam Intact female Estimated age: 4 weeks Microchip noted on Intake? negative History : Stray cat. Subjective: Underage, underweight intact female with conjuctivitis Observed Behavior – Seeking for attention Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective P = 200 bpm BCS 3/9 EENT: left eye corneal ulcer, conjuctivitis and chemosis, ears clean, no nasal discharge noted puncture wound in right side of the Jaw. Oral Exam: NAF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Intact female MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment conjuctivitis corneal ulcer Plan erythromycion opth ointment BID for 10 days amoxi-clav potash 45.7 mg/ml-0.2 cc BID for 14days Prognosis: fair.
Hx: Intake yesterday – wound L chin, corneal ulcer OS
S/O) BARH. MM pk. Appears comfortable.
EENT: OS – corneal ulcer, mild chemosis, MPOD. OD clr. NND.
MSI: Puncture R jaw – scab, mild redness and swelling, shaved. BCS= 4/9. Ambx 4. Good haircaot.
H/L: HR – 180, nsr, nm. Cl and eup.
Abd: SNP, NMP
- A) Corneal ulcer OS
Wound L chin
- P) On eye oint and oral abs.
Prognosis: Good to fair.
At front of kennel, sitting upright with ears forward. She remains in the front as I open the kennel door and allows petting along her head and body, raising her tail. Doing well!
