Medical Summary DVM Intake Exam Estimated age: 12 weeks Microchip noted on Intake? No History : Presents as stray. QAR- wobbly while walking. Nervous- allows all handling. Evidence of Cruelty seen- No Evidence of Trauma seen – No Objective P = 192 R = 42 BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: Gingivitis, mm light pink, tacky PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: intact female MSI: Ambulatory x 4, Maggots surrounding rectum / vulva, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Inflammed rectum / vulva with maggots present Assessment: Maggot infestation of vulva / rectum Plan: Recommend soaking hind end in diluted chlorhexidine BID AmoxiClav 45.7 mg/ml- 0.4 ml PO BID x 10 days If still heavy maggot infestation- recommend EHR PCV 28 TS 8.0 Prognosis: Fair to gaurded SURGERY: Temporary waiver due to maggot infestation
