ACTIVITY LEVEL: Laid back VOCAL: Somewhat chatty CHARACTER TYPE: Sweet MEDICAL BEHAVIOR: 8/7/17- Subjective: Crouches to table, allows handling, looks around, moves slowly ENRICHMENT NOTES: 8/7/17- Curled up asleep on top of den, woke when spoken to. Ate offered treats, completely ignored me. Once done eating, leaned and arched into pets, and purred. EVALUATION: Cage Condition: No change Reaction to assessor: The cat remains neutral, lying down on his cage bedding with head up during the approach. Reaction to cage door opening: The cat remains soft body in place, alert with normal shape eyes, ears erect and forward. Reaction to touch: The cat head bunts the assessor’s hand and purrs while appreciating the petting. Reaction to being picked up: The cat briefly allows the pickup and prefers to be in his cage. BEHAVIOR SUMMARY: Average Frostbite interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

Scanned negative on intake History : Stray cat, no hx available Subjective: Crouches to table, allows handling, looks around, moves slowly Objective BAR-H, MMs pink and moist, BCS 5/9 EENT: Mild mucoid discharge OU. Alopecia and mild excoriation on lateral half of lower eyelid on left eye. Clean ears. Licking nose repeatedly. Oral Exam: Moderate dental disease. PLN: No significant enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Small superficial abrasion on lateral left hock. Ambulatory x 4, large amount of flea dirt. No masses noted. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally Assessment 1. URI and conjunctivitis 2. Severe flea dirt Plan: 1. Doxycycline 1 ml PO SID x10 days 2. Erythromycin OU BID x10 days Prognosis: Good SURGERY: Temporary waiver due to URI 1088
