Medical Summary 06/04/17 DVM Intake Exam Estimated age: 1 yr Microchip noted on Intake? No. History : Litter of 3 kittens found with queen, no medical concerns noted. Subjective: Observed Behavior – Quiet, calm. Allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: BARH. mm=pink, moist, CRT<2 s. HR=200, RR=20. BCS=3/9. ORAL: All adult teeth erupted, no oral lesions. EENT: No oculonasal discharge, no sneezing. H/L: Lungs clear, no murmurs/arrhythmias. PLN: No enlargements noted ABD: Non painful, no masses/organomegaly palpated. U/G: Female. MS: Ambulatory x 4, no lameness. INTEG: Full, mildly dirty haircoat. Skin free of parasites, no masses noted. Mod. mammary gland enlargement, lactating. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Approx. 1 yr old nursing queen, mildly underweight but otherwise apparently healthy. P: Temporary waiver from OVH surgery due to nursing kittens. Excellent prognosis.
