Medical Summary DVM Intake Exam Estimated age: Reported 7 years. Exam is consistent with a 4-8 year old cat Microchip noted on Intake? Scanned negative History : Owner surrender, no health hx available Subjective: Quiet, stays crouched on the exam table. Allows all handling. Slightly tense. Objective Estimated 5% dehydrated – decreased skin turgor. MMs pale to pink. BCS 4/9. CRT < 2 sec, EENT: Eyes clear, Moderate black granular discharge. No nasal discharge noted. Oral Exam: Moderate tartar, food in mouth. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male intact cat, testicles S/S MSI: Ambulatory x 4, multiple scabs and patchy alopecia on dorsal neck. Fur coat slightly rough. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Normal externally. Assessment: 1. Dehydration – R/O thyroid disease, CKD, liver dz, other 2. Likely ear mites 3. Scabs on dorsal neck – R/O fleas vs. other ectoparasites Plan: 1. Run CBC/chemistry/T4 – all unremarkable 2. LRS 200 ml SQ SID x5 days 3. Clean ears, start Tresaderm AU BID x10 days Prognosis: Fair SURGERY: Okay to neuter. 1088
