Medical Summary S/O: HBC brought in MC negative depressed mentation bloody nares, sneezed blood M/S: no palpable mandibular or facial fractures, sternally recumbent, no palpable long bone fx’s, no spinal pain EENT: pink gums, attempted to assess PLRs- third eyeslids up, cannot assess, but pupils were mid range prior to attempting to assess H/L: increased lungs sounds bilaterally, tachypnea, RR=60, HR=180, no m/a ABN: SNP, when rectal thermomter placed, poor anal tone UROGEN: FI A: HBC tachypnea P: 2v full body rads- L caudal ilial fracture, SQ emphysema along dorsal and ventral planes of back and belly, moderate patchy opacity in lung fields-r/o pulmonary contusions, possibly rounding of caudal lung edges- r/o mild pooling of blood-hemothorax, possible caudal rib fractures buprenorphine 0.3mg/ml: 0.35ml IM, to be given 0.3ml at side of cheek BID x3 LRS 100ml SQ given monitor for urination, defecation, appetite and monitor respiration contacted NH for possible placement rec ortho consultation for fracture and hospitalization for lung contusions prognosis: open
