Initially allowed all handling, became tense during exam Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = WNL BCS 4/9 EENT: Nuclear sclerosis, ears clean, no nasal discharge noted Oral Exam: Moderate dental tartar/gingivitis, PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Female, no spay scar seen or palpated MSI: Ambulatory x 4, skin free of parasites, no masses noted, bilaterally symmetrical alopecia along both sides of body with underlying dermatiits/superficial wound along right side, matted at hind end CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Bilaterally symmetrical alopecia with dermatitis on left side, dental disease Plan: Rec CBC/Chem/T4. Prognosis: Open – pending bloodwork SURGERY: Permanent waiver due to age
