Medical Summary DVM Intake Exam Estimated age: 3-4 Microchip noted on Intake? n History : feral cat, reported not eating and ‘swaying back and forth”. observed samcking lips but unable to observe gait due to feral nature Subjective: Observed Behavior – feral, trying to bite, netted Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 120 R = wnl BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: limited. definite gingval recession but full oral not possible H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: ear tipped MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: dental disease, possible gait issues Plan: convenia 0.3ml, observe gait Prognosis: fair
