Subjective: Kitten is BARH. Eating and drinking. Normal stool in litterbox.
Observed Behavior – Kitten is at front of cage, seems friendly, but then gets low and hisses when approached. Warms up quickly and starts purring. Allows all handling with minimal restraint.
Evidence of Cruelty seen – no
Evidence of Trauma seen – no

BCS 4-5/9
EENT: Eyes chemosis OU, ears mild ceruminous debris, crusty nasal discharge and audible congestion
Oral Exam: healthy deciduous dentition; no oral lesions noted
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Appears distended; Non painful, no masses palpated
U/G: unable to palpate both testicles, but kitten moving about
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat
CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: not performed
Assessment: Conjunctivitis/URI
Prognosis:
Good
Plan: Move to ISO. Start on Doxycycline Oral – 0.4 mLs PO q24h x 10d and Erythromycin ophth oint – thin strip OU q12h x 7-10d. Recheck day 7 and 10.
