Medical Summary DVM Intake Exam Estimated age: 1-3yrs (difficult to age; teeth aren’t that bad, but cat is in poor condition) Microchip noted on Intake? yes (981020021087991) History : Return adopt due to too many health issues Subjective: Cat is very vocal in cage Q/BAR Observed Behavior – very vocal in cage, immediately social and interactive, allows handling with little to average restraint; sensitive to needle for blood draw (jumps) Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = 101.0F P = 150 R = 30 BCS 4/9 EENT: crusts outer pinna AU; OD is blue, OS is green; partial alopecia along nasal planum Oral Exam: Grade 1/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: PU/PD MSI: Ambulatory x 4, but spends most of time in lateral recumbency; coat is generally unthrifty CNS: mentation seems a little dull – no signs of neurologic abnormalities Rectal: not performed Assessment: PU/PD (USG on UA done at previous vet was 1.003; glucose and BUN/Crea wnl). Cat drank both bowls of water in cage and proceeded to urinate large amount in litterbox Plan: Performed DTM to ruleout ringworm as cause of crusting and alopecia. Recommend rescue placement with understanding cat needs thorough internal medicine workup (i.e. repeat bloodwork, urine C&S, AUS, etc.) Prognosis: Poor-Fair depending on Dx SURGERY: Already neutered

OWNER PROFILE:
Fitzgerald is a 2 year old neutered white and black smoke DSH mix; he was surrender due to is health issues of having constant urination in the home and unkown underlying health issues and is up to date in his vaccines.
Around strangers Fitzgerald is friednly and outgoing he purrs and likes to be pet. he has never been around children or any other animals and has no bite history. He will struggle to go inside the carrier. Fitzgerald was described as talkative, playful and affectionate with a high activity level.
He litter needs to be changed daily and he will have accidents daily if it is no clean. Fitzgerald is feed dry food and wet food. Upon intake Fitzgerald was relaxed and allowed the counselor to collar with out a issue.

Fitzgerald’s previous owner reports that he was urinating outside the litter box, but we are unaware of what, if any, modification steps were taken to try and address this behavior. Please note as well that Fitzgerald was diagnosed with polyuria and polydipsia (an abnormally high urine production and an increased level of thirst) which may have contributed to the behavior seen. We cannot be sure whether this behavior will continue in a different home environment, but we recommend that potential adopters be comfortable with management and modification techniques for elimination outside the litter box should this behavior continue in a home environment.
BEHAVIOR SUMMARY: Beginner
Fitzgerald interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. Although he is being treated for a medical condition, he remains social and friendly with his human caretakers. The behavior team believes that Fitzgerald can go to a home with beginner cat parents.
