Your newly adopted pet has been diagnosed with (SEVERE PELVIC INJURIES ) and the staff veterinarians are issuing a (temporary) waiver from the neuter requirements of the City of NY. Follow up care at your regular veterinarian is recommended to ensure continued treatment. Your veterinarian will advise you if surgical sterilization is appropriate.

EENT: Eyes clear, ears clean, no nasal or ocular discharge noted
Oral Exam: Mild dental staining, tartar
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated – normal bladder but painful when palpating hind end
U/G: Male
MSI: Ambulatory x 2 – dragging hind limbs, deep pain intact, normal anal tone, skin free of parasites, no masses noted, two full thickness skin wounds at base of tail, irregular shape, ~5 and 7mm, moderate swelling in that area
CNS: Mentation appropriate – no signs of neurologic abnormalities
Radiographs: No obvious fractures or dislocations, pelvic intact, skin defects in area of wounds at dorsal base of tail
Assessment: Wounds at base of tail, hind limb paresis
Prognosis:Fair
Plan: Rec simbadol 0.61ml SQ q24 x 5 days, onsior 0.41ml SQ q24 x 3 days, convenia 0.45ml SQ today. Daily recheck for 3 days scheduled to monitor condition and eliminations.

S/O: BARH. Attention seeking, allows all handling, excellent appetite. Normal stool in cage, no urine seen at this time
EENT: Eyes clear, no ocular or nasal discharge
HL: No sneezing, normal RR/RE
INTEG: Wounds at base of tail
MS: Non-ambulatory in hind end, motor appears slightly improved since initial exam
A: Nerve damage to hind end
P: Continue with current treatment and monitoring plan. Fair prognosis
