Medical Summary 04/06/17 History – Owner lost home, no medical concerns known. MN, approx 12 yrs, microchip scan neg. Subjective Observed Behavior – Shy, very calm, allows all handling. Evidence of Cruelty seen – No. Evidence of Trauma seen – No. Objective: QARH. mm=pink, moist, CRT<2 s. P = 180 R = 20 BCS = 3/9 ORAL: Severe calculus, gingivitis. EENT: No oculonasal discharge, no sneezing. Ears WNL. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias. ABD: Non painful, no masses/organomegaly palpated. U/G: Neutered male. MS: Ambulatory x 4, no lameness. Mod. generalized muscle wasting. INTEG: Full, clean haircoat, no skin lesions. CNS: Mentation appropriate – no signs of neurologic abnormalities. A: Geriatric cat, age est 12 yrs. Severe dental disease. Thin body condition – r/o renal disease, hyperthyroid, hepatic disease, neoplasia, other. P: Schedule for CBC/CHEM/T4. Fair prognosis pending on lab results.
