Medical Summary DVM Intake Exam Estimated age: 6 weeks Microchip noted on Intake? No History : Stray, came in with sibling Subjective: BAR, <5% dehydrated Observed Behavior – Hissed on exam, does not try to flee. Allows all medical handling but seems nervous/very alert. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = P = 200 R = eup BCS 3-4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: dec dentition, no lingual ulcers PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, no feces in colon, mild abd distension U/G: 2 desc testicles, small/soft bladder MSI: Ambulatory x 4, copious flea dirt, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: NP Assessment 6 w old M DSH 1. Flea dirt 2. Dehydration 3. Mildly decreased body condition Plan -MC -FVRCP -frontline spray -pyrantel and ponazuril -15 ml SQ LRS -Retro Prognosis: Good SURGERY: Temporary waiver due to age
