DVM Intake Exam Estimated age: ~10yrs Microchip noted: No Subjective: QARH Observed Behavior – Quiet, calm for exam, allows all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = WNL R = WNL BCS 7/9 EENT: Nuclear sclerosis, ears clean, no nasal discharge noted Oral Exam: Moderate dental staining, pink mm PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Spayed MSI: Ambulatory x 4, no apparent lameness, walks very slow, skin free of parasites, no masses noted, unkempt, dull matted coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric, obese, matted, quiet Plan: Scheduled CBC/Chem. CBC shows very mild neutrophilia, all other results WNL Chemistry shows very mild hyperglycemia, all other results WNL No concerns on blood work Decreased appetite noted on rounds. Adding mirtazipine 1/4 tab PO. Recheck tomorrow

Progress exam History: Intake 1/9. 1/10 given one dose of mirtazapine and CBC/Chem showed very mild neutrophilia and very mild hyperglycemia r/o stress. Subjective: QAR. Hissing and does not allow handling. Very nervous in the kennel. Has eaten almost all of his canned food but dry food untouched. No csvd. Objective EENT: Nuclear sclerosis, no nasal or ocular discharge noted H/L: Eupneic, normal RR/RE, no c/s CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Geriatric Obese Plan: Continue to monitor while at BACC Prognosis: Fair

KNOWN HISTORY:: Ernest was brought in as a stray, so we cannot speak to her behavior in her previous home.
MEDICAL BEHAVIOR:: 01/09/18 Quiet, calm for exam, allows all handling
ENRICHMENT NOTES:: 01/10/18 Lying in back of kennel, body turned away from me and head facing towards kennel wall. She turns her head slowly to look towards me when spoken to. Tolerates petting along head and body without much response. Eats a few of the treats I place down next to her. Needs more time to adjust. 01/11/18 Lying in back of kennel, body low and head facing towards the wall. She turns her head to face me when touched and begins to raise her rear when pet along the back. Purrs and sits up, leaning in for petting while pausing on occasion to smell my hand. She seems vision impaired – doesn’t notice treats until I place them right beneath her. Doesn’t acknowledge the movement of my waving hand. Doing better today! Sweet, allows all petting and appreciates attention once approached.

Reaction to assessor:: Ernest was tense and curled up in the back of the kennel with her tail wrapped at her side.
Reaction when softly spoken to:: Ernest seems unsure of her surroundings, but leans forward slowly when the assessor calls her name.
Reaction to cage door opening:: Ernest retreats to the other side of the double kennel and presses her body against the back wall.
Reaction to touch:: Ernest lip licks and seems hesitant of contact, but she slowly starts to gain confidence and warms up with attention. She gives head butts, starts to purr and arches her back when pet along her body.
Reaction to being picked up:: Ernest was tense when picked up and tries to wiggle out of the assessor’s arms.
