Medical Behavior Evaluation BLUE
Medical Summary 04/30/17 DVM exam. History – brought in with 3 other cats from same household, no medical concerns noted. Age 5 yrs. Microchip scan negative. Observed behavior – Shy, very calm, enjoys head rubs. Allows all handling. Evidence of cuelty or trauma – No. O: QARH. mm=lt pink, moist, CRT<2 s. HR=220, RR=16. BCS=6/9. ORAL: Mild calculus, dental staining. Missing R Max C, mod gingival inflammation in this area. EENT: No oculonasal discharge, no sneezing. H/L: Lungs clear, Gr 2/6 apical murmur. ABD/UG: Soft, non-painful. No masses/organomegaly. Neutered male. MS: Amb x 4, no lameness. INTEG: Full haircoat, no skin lesions. NEURO: No neurologic deficits, PLN: No enlargements noted. A: Dental disease. cardiac murmur – DDx: congenital vs acquired (HCM). P: Monitor while at BACC. Good prognosis. Rec. cardiac consult to further assess heart murmur.

Elm is a 5 year old grey domestic shorthair. He has lived with his owner since he was rescued as a kitten along with his 3 brothers. His owner is surrendering him due to his new apartment having a no pet policy. Elm has no known medical problems or injuries.
Around strangers, Elm is described as a friendly cat, often found sitting near vistors. He has lived with 2 and 4 year olds and was very gentle and tolerant of the noise and petting. He lets adults and children pick him up and hold him. He loves to play chase with the children and his brothers. He has had no interactions with other dogs. He has lived with his 3 brothers, all from the same litter, and they play and sleep together. He has not bitten a person or animal.
Elm has no behavioral problems at home but gets nervous in car rides and loud sounds. He is not bothered by being brushed, having his nails trimmed, being held, or placed in a carrier. He is described as a friendly and playful cat. He loves to sleep anywhere near his family or visitors. He will follow people and chilren around the house and will greet people at the door. He is allowed to go outdoors from time to time but will always come back after a few hours. He loves to eat tuna flavored wet and dry food in the morning and night. He has had no litterbox issues and uses an uncovered box with clumping clay. He was not given a scratching post but does not scratch walls or furniture at home.
