Medical Summary Estimated age: approx. 6 weeks Microchip noted on Intake? no History : found as stray. no known medical history known Subjective: BAR on presentation, eating well on own, vocal Observed Behavior – relaxed for entire exam. Easy to handle, social Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = not performed P = ss R = eupneic BCS = 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: deciduous teeth present PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, distended abdomen. U/G: Female intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 6 wk old FI DSH: underage and underweight to stay in shelter. Plan Patient should go out to foster until 2 mos. old Prognosis: Good SURGERY: Temporary waiver due to age/weight
