Medical Summary DVM Intake Exam 4.1 oz (116 g) Estimated age: <24 hours old Microchip noted on Intake? Scanned neg History : Finder had heard that there was a pregnant cat outside his workplace yesterday. Today he came in and found four kittens – two dead, two barely alive. One of the dead kittens had the placenta still attached, a large area of skin ripped off. The other live kitten has small lacerations to one foot and at the anus. Subjective: On arrival – not moving, breathing about 2-3 times a minute. Objective BCS 4/9, MMs pale pink EENT: Eyes closed, ears still folded. No nasal discharge noted Oral Exam: Dirt in mouth, no teeth H/L: RR 2-3/min. Not ausculted initially. ABD: Not distended U/G: Female MSI: Soaking wet, cold and covered in dirt. CNS: Minimally responsive Assessment: Hypothermic, soaking wet and dirty neonatal kitten Prognosis: Guarded Plan: Because pt was already wet, and cold and dirty – put pt under warm water to warm up and clean off. Pt began moving around and mewing weakly. Dried off pt with towel and placed on heating pad, and used hair dryer to warm pt up and dry her off. Fed KMR via 3 ml syringe. Stimulated – no urine or feces. Kept patient on heating pad, requested transport to MACC. Temporary waiver due to: underage and underweight 1088

Subjective: On arrival – not moving, breathing about 2-3 times a minute. Objective BCS 4/9, MMs pale pink EENT: Eyes closed, ears still folded. No nasal discharge noted Oral Exam: Dirt in mouth, no teeth H/L: RR 2-3/min. Not ausculted initially. ABD: Not distended U/G: Female MSI: Soaking wet, cold and covered in dirt. Right hind foot has multiple lacerations and one of the toes is partially detached. Small laceration at the margin of the anus. CNS: Minimally responsive Assessment: Hypothermic, soaking wet and dirty neonatal kitten Plan: Because pt was already wet, and cold and dirty – put pt under warm water to warm up and clean off. Pt began moving around and mewing weakly. Dried off pt with towel and placed on heating pad, and used hair dryer to warm pt up and dry her off. Fed KMR via 3 ml syringe. Pt kept mouth open with tongue protruded, and was not able to swallow effectively. Milk came out pt’s nose several times. Stopped trying to feed because pt could not swallow the milk. Stimulated – no urine or feces. Kept patient on heating pad with sibling, requested transport to MACC. Recommend euthanasia. Prognosis: Grave 1088
