SAFE 7/11/18 Eli is a black and white domestic short hair cat. He is estimated to be about ten years old. He lived with his previous owner for 3 months and was surrendered when his owners landlord told him to get rid of Eli. Although previous owner said Eli is shy he has allowed all handling in the shelter. 10 yr old ELI was surrendered by his owner and needs a furever home this time.

Oral Exam: adult dentition with moderate to severe, no oral lesions noted
PLN: No enlargements noted
H/L: NSR, Grade 2/6 HM, CRT < 2, Lungs clear, eupneic
ABD: Non painful, no masses palpated
U/G: MN
MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat but some diarrhea on coat
CNS: mentation appropriate – no signs of neurologic abnormalities

Plan: Continue to monitor while at BACC
Rec echocardiogram for HM before dental
Rec geriatric BW with placement
