I came in the shelter as a STRAY on 05/01/2017 from NY 10451, owner surrender reason stated was STRAY. I came in with Group/Litter #K17-095735.

ACTIVITY LEVEL: Moderate VOCAL: Somewhat chatty CHARACTER TYPE: Calm, Sweet, Curious MEDICAL BEHAVIOR: 5/01/17- Observed Behavior – shy/nervous; growls and hisses, initially allowed handling but became increasingly fractious and tried to lunge ENRICHMENT NOTES: 5/3/17- Still in carrier since intake, eyes somewhat dilated, seemed unsure. Sat in place when carrier rotated. Tolerated pets, but whined quietly. Sniffed treats, ate when left alone. 5/4/17- Still in carrier. Small progress today. Eyes still dilated, but overall soft posture. Can be lured part-way forward with treats. Tends to grab them one at a time, then eat them inside carrier. Hesitated when offered hand, then briefly leaned into pets. 05/07/17- Perched on top of den, but approaches the front and meows when I stand in front of her kennel. She greets by the opening when the cage door opens and looks around. She accepts petting with a somewhat tense body, but allows gentle strokes along her head and body. When interaction stops, she hops back on top of the den and lies down. 5/8/17- Sitting at the front, soft eyes and body, looking around. Chirped when spoken to, started rubbing against the bars. Leaned and arched into pets, and purred. Ate offered treats, then lay down at the front. EVALUATION: Cage Condition: No change Reaction to assessor: Eleanor looks neutral, sitting upright by the front when approached by the assessor. Reaction to door opening: Eleanor stands tall with tail up, relaxed body posture, soliciting attention. Reaction to touch: Eleanor leans into the assessor’s hand and appreciates petting on the head and body. Reaction to Being Picked up: She becomes antsy then jumps back into the cage. BEHAVIOR SUMMARY: Average Eleanor interacts with the observer, appreciates attention, is easy to handle and tolerates all petting. This cat is showing behavior appropriate for new or experienced cat parents.

DVM Intake Exam Estimated age: 2 years Microchip noted on Intake? none History : brought in as a stray but was taken to another vet prior and they said she is already spayed Subjective: QAR Observed Behavior – shy/nervous; growls and hisses, initially allowed handling but became increasingly fractious and tried to lunge Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective BCS 5/9 EENT: eyes clear, no nasal discharge AU-copious dark brown/black discharge; pruritic Oral Exam: mm pk, moist; CRT <2 sec; mild tartar; tip fracture of upper canine with no obvious root exposure PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: recently spayed-spay incision healed with mild crusting MSI: Ambulatory x 4 appears recently groomed but hair coat uneven 2 small patches of alopecia noted on R shoulder and on RF paw-woods lamp neg CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment 1. ear mites 2. small alopecic areas-r/o secondary to grooming vs ringworm vs other 3. tip fx of canine with no root exposure Plan woods lamp-negative ivermectin.1 ml SQ now and again in 2 weeks rec’d monitor tip fracture of canine-if progresses then may need extraction Prognosis: excellent SURGERY: spayed
