Medical Summary DVM Intake Exam Intact female Estimated age: 3 weeks Microchip noted on Intake? negative History : Patient is a stray orphan kitten. Subjective: 3 week old osrphan kiten having URI and conjuctivitis. Observed Behavior – BAR Evidence of Cruelty seen – None Evidence of Trauma seen – None Objective T = Not taken P = 240 bpm R = Not taken BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: NAF PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: NAF MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment URI Conjuctivitis Plan 0.1 ml doxycycline sid PO for 10 days neo poly bac ointment BID OU for 10 days. Prognosis: fair Temporary waiver due to URI and underage.
