Medical Summary DVM Intake Exam Estimated age: 4 weeks Microchip noted on Intake?: negative History : stray Subjective: BAR Observed Behavior – very friendly for all handling Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective P = 200 bpm R = 24 bpm BCS: 4/9 EENT: mucopurulent ocular and nasal d/c bilaterally. Audible upper respiratory congestion. Chemosis and conjunctivitis ou. Oral Exam: kitten dentition, no oral ulcers noted PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic but there is moderate upper resp congestion audible ABD: Non painful, no masses palpated, distended U/G: MI, unable to palpate testicles MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: Underage/underweight URI with severe conjunctivitis Plan: erythromycin ou BID x10d doxycycline 10mg/kg PO SID x10d recheck tomorrow Prognosis: good SURGERY: Temporary waiver due to URI
