Medical Summary DVM Intake Exam Estimated age: 6 weeks Microchip noted on Intake none History : found with possible injury Subjective: stray Observed Behavior – frightened, tries to flee Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective generalized tremors, intermittent ataxia P = wnl R = wnl BCS 5/9 EENT: dried ocular discharge, ears clean, crusted nasal discharge noted Oral Exam: deciduous dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female MSI: Ambulatory x 4, will crough aand walk with ataxia, then speed up and jump/run skin free of parasites, no masses noted, healthy hair coat CNS: generalized tremors, no lameness Rectal: not performed Assessment CNS insult r/o transient – inflammatory infectious vs trauma Plan no processing today ( hold on vaccines, deworming, micro chip , combotesting) recheck tomorrow Prognosis: fair SURGERY: needs to be determined after reassessment
