Medical Summary DVM Intake Exam Estimated age: 7-9 weeks Microchip noted on Intake? no History : stray kitten; unknown Subjective: Observed Behavior – crouched low in box, observes surroundings when taken out, allows all handling, but not exceptionally social Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = NA P = 200 R = 40 BCS 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: healthy deciduous dentition PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, slightly distended; intestines palpate rope-like (suspect from intestinal parasitism) U/G: testicles small, but palpable MSI: Ambulatory x 4, skin free of parasites, no masses noted, dirty, but otherwise healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Of age (8 wks), but slightly underweight (1.9 lbs), otherwise apparently healthy Plan: Recommend foster placement until kitten is over 2 lbs and acceptable candidate for elective sx Prognosis: Excellent SURGERY: Temporary waiver due to underweight
