This sweet six month old girl is emaciated, has a URI, and needs follow up medical care for her abscess. Diddy Dee must have placement by 5pm and leave for local vet by 6pm tonight!

02/02/17:
Microchip scan neg.
Female, approx. 6 mos.
O: Lethargic, responsive. mm=lt pink, tacky, CRT=2 s. Allows all handling.
ORAL: Severe lip ulceration and swelling with purulent discharge and crusting right side. Ulcerated tongue. Large right sided hard palate defect or ulceration. Painful on oral exam. Difficult to assess dentition due to pain on opening mouth, but appears to have adult dentition, with recently erupted canine teeth.
EENT: Eyes crusted shut. Severe purulent nasal discharge.
H/L: HR=160, RR=32. Mild increased bronchovesicular sounds.
ABD/UG: Female. Palpation WNL.
MS: Emaciated body condition. Not attempting to walk, but able to rest in sternal.
INTEG: Fluctuant swelling right lat cervical region, purulent discharge draining from wound in area.
Rough, dry haircoat. Markedly dec. skin turgor.
A: URI. Oral and lip ulcerations with abscessation. DDx: viral infection (Calici, felv, fiv, other), bacterial infection, other. Dehydration 7-10%.
P: Convenia 80 mg/ml 0.15 ml SQ
Simbadol 1.8 mg/ml 0.15 ml SQ
Cleaned eye, face with warm water. Cleaned wound R lat cervical region with chlorhexidene.
Terramycin oint OU BID x 10 days.
Pradofloxacin 25 mg/ml 0.3 ml PO q 24 hours x 7 days.
SQ LRS 25 ml BID x 5 days.
Offer soft food only.
Prognosis guarded.
02/03/17:
Monitor condition
S/O: Dull, 8-10% dehydrated, no interest in food. BCS 2/9
EENT: Lip, gingivial and lingual ulcerations with dried puruelnt discharge, pale tacky mm, sunken eyes with mucoid ocular discharge, mucoid nasal discharge, painful on oral exam
HL: No murmurs/arrhythmias, slightly increased bronchovesicular sounds
ABD: Soft, non tender
INTEG: Unkempt hair coat with dried discharge at front limbs from grooming face
MS: Weakly ambulatory
UG: Female
A: Emaciated and anorexic with severe oral ulceration and resolving abscess from right cervical neck
P: Adding nebulization q12 x 3 days and buprenex 0.1ml q12 x 5 days. Continue to monitor throughout day,
poor prognosis
