DVM Intake Exam
Estimated age: 4-6wks
Microchip noted on Intake? no
History: Found in box with 2 other dead kittens.
Subjective: Kitten is QAR. Eating [well] on own once she figured out the food was in front of her.
Observed Behavior – Resists restraint for cleaning of eyes/nose, but allows all other body parts to be touched and ate food during blood draw.
Evidence of Cruelty seen – no
Evidence of Trauma seen – no
T = NA P = wnl R = wnl BCS 2-3/9
EENT: ears clean, bilateral mucopurulent ocular and nasal discharge; chemosis OU making visualization of corneas difficult.
Oral Exam: healthy deciduous dentition
PLN: No enlargements noted
H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic
ABD: Non painful, no masses palpated
MSI: Ambulatory x 4, fleas, no masses noted, coat slightly unkempt
CNS: mentation appropriate – no signs of neurologic abnormalities
Rectal: not performed
Assessment: Underage/Underweight, URI, Dehydration
Plan: Move to ISO since eating well on own. Monitor hydration status. Start erythromycin ophth oint – thin strip OU q12h x 10d and Doxycycline liq. – 0.1 mLs PO q24h x 10d.
SURGERY: Temporary waiver due to underage/underweight and URI.
