ACTIVITY LEVEL: Mellow VOCAL: Somewhat chatty CHARACTER TYPE: Social, Sweet, Playful MEDICAL BEHAVIOR: 7/26/17 Resting in litter box, sat up and became alert when door opened. Came forward for treats, tolerated or slightly leaned away from pets while eating. Hissed once, then continued eating treats. 7/27/17 Asleep in top cubby, woke when door opened, soft eyes and body. Came to the front for treats, head-butted, leaned and arched into pets, and purred. Continued soliciting attention after finished eating. EVALUATION: Cage Condition: No change Reaction to assessor: Creston immediately comes soliciting at the front of the cage. Reaction to door opening: Creston remains standing at the front of the cage, relaxed body posture, soliciting attention. Reaction to touch: Creston head-butts the assessor’s hand and appreciates petting on the head and body. Reaction to Being Picked up: Allows the pickup and remains calm. BEHAVIOR SUMMARY: Beginner Creston interacts with the Assessor, solicits attention, is easy to handle and tolerates all petting. This cat can go to a beginner home.

DVM Intake Exam Estimated age: 10-12 months Microchip noted on Intake? History : brought in as a stray; appeared as if he could not use his hind limbs Subjective: QAR Observed Behavior – timid/nervous; loves being pet but scares easily; reluctant to walk but seems behavioral; no lameness when forced to walk on a harness Evidence of Cruelty seen -none Evidence of Trauma seen – none Objective BCS 5/9 EENT: Eyes clear, no nasal discharge; AU-dark discharge, pruritic Oral Exam: mm pk, moist; mild hypersalivation; adult dentitia with mild staining on canines PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact; 2 scrotal testicles MSI: reluctant to walk, prefers to lie on side but seems behavioral; when forced to walk with a harness, no lameness is noted there is a very small, superficial abrasion between digitis III/IV on LF with small amnt of dried blood but no other obvious injuries; normal ROM at all joints, no swelling or instability CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment Ear mites Plan Gave 0.1 ml ivermectin SQ, repeat dose in 2 weeks monitor for lameness no treatment for small interdigital abrasion at this time Prognosis: good SURGERY: Okay for surgery
